The Risk Management Division processes personal injury, property or vehicle damage claims filed against the Town of Apple Valley. The terms or acceptance of claim forms by the Town is not an admission of any fault or liability on the part of the Town or any officer, agent, or employee thereof.

If you or your insured has caused damage to Town property, please contact the Town of Apple Valley Public Works Department at 760-240-7000, ext. 7500. Once the cost estimation is assessed by the Public Works Department and other appropriate parties if necessary, it will be forwarded to the Risk Management Division to process accordingly.
To file a claim against the Town, you must complete a General Liability Claim Form. The Liability Claim Form can be downloaded, emailed or mailed to you. Forms are also available on a walk-in basis. The completed form must be returned in person or by mail to the attention of the Town Clerk's Office at the address listed below.

The claim form cannot be faxed or e-mailed as the Town must have the original signature of the individual making the claim. To have the form mailed or emailed to you, please contact the Risk Management Division at 760-240-7000, ext. 7800.
As with all claims, please make sure to provide any information you believe will support your claim (i.e. estimates, photographs, diagrams, etc) at the time of submittal.
State law allows a 45-day period to conduct the investigative process. A staff of professional claims adjusters through our self-insurance pool, investigates all claims and evaluates their merits under the applicable law. As soon as the Risk Management Division has all the necessary information, you will be advised if your claim is approved or denied. The Town must have the original signature of the individual making the claim. If a claim is denied, the claimant is provided with a written response outlining the remedies allowed by state law.
